Shade Munro's side ended the tournament with one win from five, sitting above Wales in the final standings.
On a terrible, muddy pitch, Italy took a 19-0 half-time advantage with tries from Melissa Bettoni, Maria Magatti and Giada Franco.
Beatric Rigoni extended the hosts' lead, before Scotland crossed through Lana Skeldon and Eilidh Sinclair.
The Italians finish in fourth place, having claimed two victories.
