Its report found safety at Oaklands Hospital was not a sufficient priority with standard procedures for operations not always followed.
It also said patients were at risk because on some occasions, anaesthetists did not provide them with the expected level of care and that most theatre staff did not have the right level of training to react to a respiratory or cardiac arrest.
Ramsay Health Care UK which runs the hospital said a new management team was put in place immediately after the inspection and that fixing the issues was an absolute priority.
It also said significant improvements have been made since the time of the inspection.

Maureen says she met David Lytton in 1968. She was suffering from flu at the time but had ventured out to Finchley in north London to buy a stereo. It was the Last Night of the Proms and she wanted to enjoy listening to it at home.
"I didn't feel very well. I was on my knees, and I was collapsing. There was a young man who went 'Oh, hang on, hang on I'll come over,'" she remembers.
"He walked home to my flat and he made me a nice cup of tea. We hit it off. He made me some toast - I hadn't had any breakfast and he stayed with me until my flatmates came home."
Stabbing victim John Culshaw's remains were retained by Greater Manchester Police without his family's knowledge.
Dozens of other families have also been told their loved ones' body parts were kept by the force which has admitted there could be more body parts unaccounted for.
GMP said whether or not to contact the affected families was a decision it "agonised over for a number of months".
It became illegal in 2006 to remove or store human tissue without families' consent following an investigation at Liverpool's Alder Hey hospital in 1999.
