    BBC Local Live

The Ministry of Defence has been criticised after the death of a driver, crushed by a reversing lorry, at their site in Southam.

Ministry of Defence site in Kineton

The Health and Safety Executive (HSE) issued a Crown Censure, the maximum sanction for a government body, after agency driver Graham Wood was caught between the lorry and another stationary vehicle at MoD Kineton.

The MoD failed to assess the risks faced by the 55-year-old from Bicester, as he delivered goods to a holding area in November 2013, the HSE said.

The censure doesn't carry a financial penalty, but is an official record of a failure to meet standards in the law.
