Over 1,000 NHS serious medical errors in the past four years

More than 1,000 NHS patients in England have suffered very serious medical errors in hospitals over the last four years.

An investigation by the Press Association found examples of patients with medical instruments being left inside them, and body parts being removed by mistake.
