NHS 'did not investigate my son's death'

More evidence is emerging of failures to investigate unexpected deaths in the NHS.

Last week the government ordered a review of how investigations are handled, after sharp criticism of one of the biggest mental health trusts in the country.

Now, more families are coming forward to voice their concern.

Michael Buchanan reports.
