Blokhuijsen clocked 6:15.71 with Jorrit Bergsma recording 6:16.66, while Belgium's Bart Swings missed out in fourth.
Kramer also won a silver in the event at the Turin Games in 2006.
It was his 16th consecutive victory over this distance, shaving nearly four seconds off his previous Olympic record, and he remains unbeaten since 2012.
