Middlesex beat Worcestershire by five wickets to leave the New Road side 18 points from safety with two games left.
The hosts began the final day 61 ahead on 44-3 but they slumped to 186 all out, Alexei Kervezee making 64 not out.
Toby Roland-Jones (5-46) returned the first 10-wicket match haul of his career finishing with 10-118.
Needing 204 to win, Sam Robson (35) and Joe Denly (48) gave Middlesex's run chase a solid start, Gareth Berg's unbeaten 26 helped see them home
