Middlesex batsman Neil Dexter fell 10 runs short of a 10th first-class ton of his career on day three against Worcestershire.
After Sam Robson made 72, Dexter (90) led Middlesex from 181-5 to help them post 306 all out.
Alan Richardson (4-62) took his tally to 51 Championship wickets this season.
Relegation-threatened Worcestershire closed on 44-3 in their second innings, a lead of 61 runs, with Matthew Pardoe unbeaten on 22.
