A regional judging panel, made up of a cross-section of BBC West sports staff and other local sports experts, will decide the winner of the BBC West Sports Unsung Hero 2009.

All 15 BBC regional winners will then be invited to this years BBC Sports Personality of the Year show, broadcast live from the Sheffield Arena on Sunday, 13th December on BBC One, where an overall winner will be announced.

Previous overall winners

Margaret Simons was the 2007 winner. She has run Bardwell Football Club in Oxfordshire since founding it in 1964.

Ben Geyser is the driving force behind Dorchester Boxing Club. He won the award in 2008.

Don Cannon - Bowmoor Sailing Club

Last year's BBC West winner was Don. Without his efforts the club would not exist. Anything up to 50 boats take to the water each Sunday.

Every year about 50 to 60 youngsters and 200 to 300 adults take part.

Don negotiated a grant of £75,000 and was involved in raising a total of £500,000 which helped build a new clubhouse, boat house and trailer park.

To enter, you must ensure all the sections are filled in. Any nomination that doesn't include details of the nominee cannot be accepted.

Your nominee must be informed and have agreed to being entered, nominees may be asked to appear on BBC TV and/or radio.

The judges' decision is final and no correspondence will be entered into regarding their decision.
