US BMX rider Brooke Crain crashed out of her seeding run after clipping her rear wheel on a bump. She smashed her chest into the next hill but managed to climb back on and finish.
American runner Manteo Mitchell has revealed he ran the last 200m of the Olympic men's 4x400m relay heats with a broken left leg. He says he kept running because he didn't want to let his team down.
Defending Olympic weightlifting champion Matthias Steiner failed to retain his title after being crushed by his 196kg bar-bell. The 29-year-old German got stuck after dropping the weights on top of himself.
Cuba's Lazaro Borges snapped his pole as he attempted to clear 5.35m in the men's pole vault competition. Borges was not injured and immediately got back on his feet.
Stephan Feck exited the men's 3m springboard competition after his dive went wrong. The German's foot appeared to slip off the side of the diving board and he landed in the water on his back.
Turkish athlete Merve Aydin struggled to complete heat two of the women's 800m. She limped round the track after injuring herself early on in the race. After crossing the line she had to be taken away in a wheelchair.
Russia's Ivan Ukhov overcame a last-minute kit crisis to claim gold in the Olympic high jump. Moments before he was due to jump, Ukhov lost his competition vest and had to complete his jump in a T-shirt (bottom).
