BBC Scotland has uncovered new evidence about what is going wrong in our hospitals.

BBC Scotland Investigates: How safe is your hospital? asks whether we can trust the NHS to do the right thing when mistakes are made.

After a freedom of information request, we were given 345 critical incident reports, including 105 deaths. They are all published here.
