The murder of a teenager strangled by her boyfriend after he tied her to a tree exposed failings in the local care system, a report has concluded.
Hannah Windsor, 17, was killed on Bidston Hill near Birkenhead by Adam Lewis, 18, on 19 May last year.
Wirral Local Safeguarding Children's Board (WLSCB) said in a serious case review the risks of her relationship with him had not been fully explored.
However, it concluded agencies involved could not have foreseen her murder.
Hannah, from Birkenhead was found dead the day after she was strangled. Her body was found in undergrowth next to the Bidston Hill Observatory.
Boyfriend jailedThe report into her case highlighted a number of areas where the work of agencies involved were sub-standard.
Dennis Charlton, chair of the WLSCB, said recommendations in the review were now part of an action plan to improve the care system in Wirral and it would be reviewed and updated regularly by the board.
He said: "We are already working on a number of areas which have been identified, where assessments, multi agency work and information sharing were not of a sufficient standard to provide a framework for co-ordinated actions and interventions.
"The results will contribute to child protection practice in Wirral and will be used to inform further action and training."
Lewis, of Fairmead Road, Moreton, Wirral, was jailed for life for Hannah's murder in November.
He had denied murder but changed his plea before his trial was due to begin at Liverpool Crown Court.
Mr Justice Holroyde ordered him to serve a minimum term of 22 years.
