Lee Balkwell concrete mixer death inquiry 'flawed'

An investigation by Essex police into the death of a 33-year-old man in a concrete mixer was seriously flawed, according to findings written in a leaked report.

Lee Balkwell, from Hornchurch, was crushed on a construction site in Upminster in 2002.

Essex Police said it was an accident but his family believed he had been murdered.
