29 April 2014 Last updated at 11:40 BST

A coroner has recorded a verdict of suicide at the inquest into the death of an Essex teenager who was found dead at a children's home.

Rebecca Watkins, 16, from Braintree, was found hanged at the Evergreen Children's Home in Brandon, Suffolk in 2009.

At the inquest in Bury St Edmunds, Suffolk coroner Peter Dean said there had been a "significant catalogue of failings" and had been "failed time and again".

Essex County Council said it "deeply regrets its failure to identify opportunities in the care of Rebecca... which may have made a difference in her life".

Kim Riley reports.
