Stafford neglect: Why the delays?

Following the deaths of hundreds of patients, a report on Mid Staffordshire Hospital blamed a culture that put targets and cost-cutting before care.

For many relatives, there is still one major question - why did it take so long to uncover significant levels of neglect?

The BBC's Inside Out programme examines why it took so long to uncover the neglect at the hospital.
