Failures in NHS standards exposed by watchdog
More than a quarter of care homes and NHS hospitals in England are failing to meet legally required standards of quality and safety.
Thousands of spot checks were carried out unannounced.
The Care Quality Commission says it helped them build up a comprehensive picture of patient care.
