EU health insurance cards: How should they work?

Spain is facing legal action from the European Commission following complaints that British holidaymakers have been charged for emergency treatment despite having a European Health Insurance Card.

All EU citizens have the right to receive state medical care in member countries if they produce the card, but increasing numbers of people say they've been asked to pay for treatment.
