Lessons to be learned over Gwynedd family killings

Lessons need to be learned from the case of a man who killed his wife and two-year-old son at their Gwynedd home, an official review has found.

David Wyn Jones, 42, stabbed them to death, near Tremadog, in March 2012.

He later admitted manslaughter on the grounds of diminished responsibility and is being detained indefinitely.

A serious case review suggested health professionals, the police and social services should have been quicker to share information about his deteriorating mental health.

However, it concedes the outcome may not have been different if they had.
