A surgeon who failed to tackle internal bleeding quickly enough in a patient who later died has been placed under supervision for a year.

Pembrokeshire businessman Malcolm Green suffered complications after an operation for bowel cancer at Withybush Hospital in Haverfordwest.

Hywel Dda University Health Board has already admitted failings in his care.
