RAF death of Scott Hughes 'avoidable'
A coroner has blamed a systemic failure of safety systems after an RAF airman died when he was struck by a speedboat in Cyprus.
Senior Aircraftman Scott Hughes, 20, from Y Felinheli in Gwynedd, had just completed a tour of Afghanistan.
His family said his death was a "wholly avoidable catastrophe". They said they are planning to sue the Ministry of Defence.
Matthew Richards was at the inquest in Llandudno.
