Kyle Keen: Toddler death 'could have been avoided'
A report has revealed the death of a toddler could have been avoided if hospital staff had alerted social services to suspected abuse.
Kyle Keen died aged 16 months at Walsall Manor Hospital in 2006 after being shaken by his stepfather and suffering severe head injuries.
A week earlier he had been admitted with severe bruising but doctors and a nurse who wanted to report his injuries were overruled.
