Mother and toddler were 'at high risk of homicide'

The partner of a pregnant woman, who was fatally stabbed alongside her 23-month-old son, has said their deaths could have been prevented.

A Serious Case Review published on Thursday says the deaths of Rachael Slack and her child, Auden, could not have been prevented or predicted.

But Rachael's partner, Robert Barlow, said different agencies should have done more to make her aware of the risks posed by her former partner.
