I closed down a manufacturing plant for a day after I released some new code into their business system. Even though I had tested the b*****y out of the module in the test environment, when I put it live the whole system came crumbling down. Senior managers were flying around the place and I spent the day drinking coffee and looking a very jaundiced pale green. Not only did the company close down, so too did all my thinking faculties. I was saved by a colleague who went on to be my best man years later. Net cost of downtime - about 3 million euro....
